We have recently reviewed our process for collecting essential medical documents for students with diagnosed medical conditions, including Asthma, Allergies, Anaphylaxis, Diabetes, and Epilepsy. It is crucial that these documents are updated annually or whenever there are changes to your child's condition.

These required documents include a Medical Action Plan completed by your doctor, and a Risk Management Plan, which outlines your child’s specific needs and is completed by you.

To make this process more efficient, we have introduced a new online system that allows you to easily upload these important documents. You can access this system by logging into the Parent Portal on SEQTA, selecting the "Forms & Medical" tile, and then clicking on the "Medical Management Plan" tile.

We kindly ask that you ensure these documents are kept up to date to help us provide the best care and support for your child.
