Year 10 Secondary School Vaccine Program

Dear Parents/Guardians,

As per the previous communication on Tuesday 11 February 2025, the Secondary School Immunisation Program offers free Meningococcal ACWY vaccines to all eligible Year 10 students.

The Department of Health will collect your personal and health information to complete your registration in the Central Immunisation Records Victoria (CIRV), create a record of your vaccinations, and obtain your feedback after the vaccination.

The department will use this information to:

  • book the vaccinations;
  • provide you with a reminder about the bookings;
  • provide you with a reminder about when the next vaccination is due.

Failure to provide complete information during the registration process or in response to the survey may hinder an accurate assessment of your child's suitability for the vaccine and/or the identification of any adverse reactions, potentially posing a risk to their health.

Enter mobile number – click “NEXT”.

Enter 6-digit code sent to your mobile – click “NEXT”.

Check your child’s school details at the top of the screen are correct. If yes, continue through the next steps which include information on the immunisations and box to tick to confirm consent.

Please be aware that fields marked with 'Required' must be filled out. Kindly ensure that the online consent form is submitted by Sunday 30th March.
